Whether you are a physician treating a patient or you are a patient yourself, it is essential to advise/take the right Contrave dosage.

Taken in the right form and at the right/scheduled time, the drug will allow you to get the maximum positive effect. Contrave will not yield desired effects if you are not taking the Contrave dose prescribed by your physician.

Contrave is available as the extended release, film coated tablet form with a strength of 8 mg naltrexone hydrochloride + 90 mg bupropion hydrochloride per tablet. The Contrave tab is debossed as NB890 at one side. Contrave Packaging is available in 70 & 120 tablet bottles. The drug instruction advises storing the pills at 59 to 86⁰F.

You should know about complete dosage regime of Contrave before starting obesity treatment. Titration is the right way to administer the drug, i.e. Contrave dosing gets a gradual increase at the start of the treatment, to get optimal weight loss benefits with minimal to no side effects.

On the first week of treatment, Contrave dose is one tablet in the morning only. On the second week, the dosing should be one tablet in the morning and one in the evening. The third week, you should take two tablets in the morning and one in the evening. From the fourth week onwards, the patient should consume two tablets each morning and two every evening (maintenance dose).

This maintenance dose should continue till the time medicine is affecting the weight loss with no significant side effect. It shows weight loss effects after 12 weeks of regular intake. However, if it shows no desired result after 12 weeks, then medication needs to stop. Various clinical studies show that Contrave grants significant 8% weight loss if taken for 52 weeks.

While taking Contrave for weight loss, you must take low-fat, low-calorie diet and follow an exercise routine to get maximum benefits.

If you are suffering from other diseases along with obesity, the dosage will differ from the one above. You need some adjustments/modifications to avoid any risk & get optimum benefits.  Below is a list of the dose adjustments:

  • Renal impairment:  If you suffer from any severe or moderate renal disease or condition, you should reduce Contrave dosing to a maximum of two tablets per day. The Contrave diet pill is not advised for patients with end-stage kidney diseases.
  • Hepatic impairment:  If you are suffering from liver impairment, maximum recommended dose is one tablet per day.
  • Use with CYP2B6 inhibitors: If Contrave dosing is continued with CY2B6 inhibitors, maximum dose should be two tablets per day.
  • MAO inhibitor: If you are taking MAO inhibitor drugs (Antidepressants) or planning to take while already on Contrave, the minimum 14 days’ gap should not be kept. Start Contrave dosing 14 days after stopping antidepressant or start antidepressant 14 days after stopping Contrave dosing intake.

To analyze the overdose risk of Contrave intake, clinical trials conducted with a maximum dosage of 50 mg naltrexone and 400 mg bupropion. Mostly overdose effects were due to bupropion. Seizures were observed in one-third of total subjects with overdose. Unconsciousness, hallucinations, tachycardia, ECG changes (QRS prolongation), muscle rigidity, shortness of breath are some overdose effects observed. Though almost all patients recover with overdose, there are reported deaths of a long dose of bupropion alone.

If overdosing happens, you should contact your physician or emergency medical center/poison control center. There is no antidote to Contrave. The patient should be kept under strict medical observation and monitoring and provide continuous supportive care. Do not induce vomiting. Airway, breathing, and ventilation must be maintained. Vital signs should be checked at frequent interval.

When you are taking Contrave to treat obesity, you should always be aware of its effectiveness, tolerability, and safety concerns. You should listen to your body and act accordingly.

To measure the effectiveness of Contrave, various clinical studies have been conducted in obese and overweight patients who are already suffering from hypertension, hyperlipidemia, and diabetes. As a result, it is observed that, when Contrave is given in appropriate dosage to these patients for 12 months, they showed significant 5% more weight loss as compare to the patients with placebo. Though it is not known whether patient gains weight post withdrawal of Contrave dosing.

If we talk about the tolerability of Contrave dosage, GI (gastrointestinal) symptoms such as nausea and constipation are common in almost 19% patients.  Sleep disorder, dizziness, anxiety, and headache, high blood pressure are other common symptoms when the patient is taking Contrave dosages. Some major side effects of taking Contrave are a Blurred vision, fear, nervousness, slow or fast heartbeat, mania, visual impairment, tiredness, and irritability. You should pay attention to these symptoms when on Contrave dosage and if found any, consult your physician to further decide the line of treatment.

It is always better to identify a tolerability problem early so that appropriate measures can be taken right away. There are serious side effects of Contrave like suicidal thoughts and suicidal actions.

Patient safety should always be taken into account when prescribing Contrave. There are known the risk of seizures and opioid withdrawal symptoms to appear when a patient is taking Contrave.  Contrave is not indicated for patients with uncontrolled hypertension and pregnant/breastfeeding females. Also, it is a drug for adults and not recommended for children and adolescents suffering from obesity.

It is quite common that you are taking certain medication for some other ailment or took any over the counter(OTC) drug for time being. There are certain drugs and disease conditions which interact with Contrave ingredients and can cause harmful effects on you. Contrave should not be taken with alcohol and certain disease conditions like cardiac diseases or if you are taking antidepressant drugs.  A physician needs to know all your medical history before prescribing Contrave so that undesired interactions can be avoided. You should also be aware are allergic to and should inform your physician before starting Contrave dosage.
